Coastal Plains Integrated Health provides community services to persons with intellectual and developmental disabilities.  All services are based on individual needs and designed for people to live and work successfully in their communities.  Our focus is on community-based services with an emphasis on job skills training to prepare people for competitive community employment. Coastal Plains Integrated Health currently operates under the guidance and direction of a nine member Board of Trustees.  Each of the nine counties served by Coastal Plains is represented by one board member.  The nine counties served by Coastal Plains are Aransas, Bee, Brooks, Duval, Jim Wells, Kenedy, Kleberg, Live Oak and San Patricio.

How Do I Begin? To access services call the Mental Health Clinic nearest you. You will be given an appointment for an assessment to be conducted by a diagnostic team.

What Is The Cost Of Services? All fees for services are charged according to a sliding scale based upon the family income and size. Fee Schedule

Coastal Plains Integrated Health provides mental health services to adults with severe and persistent mental illnesses such as schizophrenia, bipolar disorder, and severe clinical depression. Crisis services are provided 24 hours a day, seven days a week.  These crisis services may be accessed through a Crisis Hotline at 1-800-841-6467. All information about services received is strictly confidential.

Adult Mental Health Services >

Intellectual & Development Disabilities is a clinical diagnosis defined as a usually permanent condition originating sometime between birth and age 18.  The person’s general intellectual functioning is significantly below average (roughly an IQ of 70 or below) and his or her behavior does not meet the level of personal independence and social responsibility expected of the person’s age and culture.

IDD Services >

Coastal Plains provides community-based services to children/youth ages 3 through 17 with a diagnosis of mental illness (excluding a single diagnosis of substance abuse, IDD, autism, or pervasive developmental disorder) who exhibit serious emotional, behavioral, or mental health disorders.  If your child or adolescent qualifies for services, he or she will be placed in a level of care which will determine the services your child will receive.   Our Center strives to provide the most appropriate and beneficial services, however, in order to be successful the treatment process requires active participation from both the parents and child.  Crisis services are also available for child/youth 24 hours a day, seven days a week. The Veterans Services and Supports Project will assist local veterans and their families in accessing resources available within the community including food, housing, counseling, health care, job searches, etc. We will also assist veterans in linking up with  local and statewide resources such as Disabled Veterans, “Grace Under Fire”, American GI Forum and others which assist veterans in obtaining benefits and other services.

Coastal Plains Integrated Health in conjunction with United Connections Counseling are working together to provide substance abuse services for Rockport, Aransas Pass, Taft, Beeville, Falfurrias, Alice, and Kingsville. We will even help you with finding a way to get to the groups.  Meet with your case manager to find out what we can do to help you get the treatment you need to become healthy.

Coastal Plains Integrated Health’s Integration Program makes quality primary care services available to consumers who may have no other access to medical care.  Primary care and behavioral health services are now conveniently available under one roof in all Coastal Plains clinics.

TCOOMMI ICM program is designed for adult offenders currently on probation having been referred to the program from their current correctional officer.

TCOOMMI SNDP is designed for juvenile offenders currently on probation having been referred to the program from their current correctional officer. 

The COC program is designed to conduct pre-release screenings and referrals for post release identification of medical, psychiatric, psychological care or treatment, educational, or rehabilitative service needs referred from TDCJ CID and other correctional offices.
